For New Patients Looking to Pre-Register

 For potential patients looking to pre-register and bypass filling out all the paper work in the waiting room, please follow these instructions:

  • Print & download New Patient Form Worksheets, then read and sign all pertinent pages.
  • Proceed to Metro Spine Patient Portal Pre-Registration page and fill out all the necessary information, and when completed click submit.
  • Follow up with our office (301) 856-5860 to notify us that you've pre-registered yourself to the Metro Spine Patient Portal and would like to schedule an appointment.
  • Bring the New Patient Form Worksheets with you to hand in when you come in for your initial visit.
